Tip: Preserve SERVER_NAME and HTTP_HOST

Script which rewrites $_SERVER["REMOTE_ADDR"] etc to use the HTTP_X_FORWARDED_HOST handed over by nginx and other proxies. Sorts out various odd issues with wordpress.
